How do insurance companies (such as AFLAC) find out about pre existing conditions?

How do insurance companies discover pre existing conditions? Do you just have to wait the waiting peirod (usually 6 months) to be allowed to file a claim or will any chronic illness be exempted for life if it was diagnosed prior to joining the plan?

Due to HIPAA (health insurance portability and assurance act) insurances can access health information. I do believe in order to do this than they need a patient release form from you signed. They’re a business so they want to make sure they make money. I would research the companies and their policies before signing on…

    The doctors of the insurence company’s have there own methods,
    they can check by so many methods.
    They spy on your medical records anywhere.
